Evidence of Insurability (EOI) – Some benefits may require EOI, based on the amount of coverage you elect. The EOI form requires information regarding medical history for you and/or the dependent you are enrolling. Based upon your responses to these questions, the carrier may reach out to your medical provider for additional information. The carrier reviews this information, and their underwriting department makes a determination as to your eligibility to enroll in the benefit. They determine who is approved/not approved for coverage. You will not be charged a premium until the carrier has made a determination as to your eligibility for coverage. EOI forms are provided under Plan Documents (below). Complete the EOI form and send to the carrier listed on the top of the form.
Benefits requiring Evidence of Insurability (EOI) during New Hire Enrollment:
- Voluntary Life (Securian) – ALL Elections will require EOI
- Long Term-Disability (Madison National Life) – if you work less than 20 hours/week OR if you chose to elect coverage during Open Enrollment EOI is required.
Guarantee Issue (GI) – Some carriers offer GI, meaning you are automatically covered for the benefit, without the requirement to provide any medical history. Typically, GI is offered if you are enrolling in a benefit during an initial enrollment period (with a new carrier) or during a new hire enrollment. Once you have been offered GI, the carrier may not provide it again during a subsequent enrollment period.

- Basic Life (Securian)
- Child Voluntary Life (Securian)
- Long-Term Disability (Madison National Life) – Provided you work at least 20 hours/week and making an election when you are 1st eligible as a New Hire.
- Accident (Symetra)
- Critical Illness (Symetra) – Coverage up to $10,000, $20,000 or $30,000
